These terms apply when you use the New Promise Neuropathy website, request an appointment, complete a form online, or use the patient portal. By using the website you agree to them. If you do not agree, please do not use the website.

Medical Information and Emergencies

The information on this website is general and educational. It is not medical advice, and reading it or contacting us through it does not make you our patient. A patient relationship begins only when one of our providers evaluates you. If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use this website or its forms for urgent medical needs.

Appointment Requests and Online Forms

• An appointment request is not confirmed until our office confirms it with you.
• Please give accurate and complete information, including your insurance details. Services, providers and coverage vary by location and plan, and insurance must be verified before treatment.
• When you submit a form you agree that we may contact you about your request by phone, text message or email. Message and data rates may apply. You can reply STOP to any text message to stop receiving them, or ask our office to change how we contact you.
• Forms you complete online, including consent and intake forms, may be signed electronically. An electronic signature on our forms has the same effect as a handwritten one, and you may ask our office for a paper copy of anything you sign.

Patient Portal

If you have a patient portal account, keep your sign-in details private and tell our office right away if you think someone else has used your account. You are responsible for activity under your account. Your health information is handled as described in our Notice of Privacy Practices (HIPAA), which controls if it differs from these terms.

Our Privacy Policy explains what information the website collects and how it is used.

Acceptable Use

Please do not misuse the website. That includes submitting false information or information about someone else without their permission, trying to gain access to accounts or systems that are not yours, interfering with the website's operation, or copying its content in bulk by automated means.

Content and Trademarks

The text, images, videos and logos on this website belong to New Promise Neuropathy or are used with permission. You may view and share pages for personal, non-commercial use. Any other use requires our written permission.
